Next, you'll need to register with the California Department of Social Services Guardian system.
This registration requires a $35.00 fee, payable directly to the Department of Social Services. Once paid, your Guardian registration will be active for two years, after which renewal is required.
This is a mandatory step for all caregivers in California. Once registered on Guardian, you'll receive a pre-filled Live Scan form.
IMPORTANT: You will need our Agency PIN Number. See below for 'Registration Type'.
PIN: UPPTPC

A ‘Registration Type’ will need to be selected.

Once registered on Guardian, you'll receive a pre-filled Live Scan form.
To complete your Live Scan background check, you'll need an authorized Live Scan provider.
Use the California Department of Justice website to find a provider near you. Follow the link provided below and enter your city, or a neighboring city, to view a list of providers.
Please note that the background check involves a fee. This fee is charged by the Live Scan provider and will vary from location to location. We recommend contacting your chosen provider to inquire about their specific fees and to schedule an appointment.

Finally, you will need to provide proof of a recent TB test. You can get tested at your doctor's office, an urgent care clinic, or a pharmacy.
When you go, simply ask for a TB test for work; the doctor, clinic, or pharmacy will provide you with the necessary form
If you have already completed a TB test within the past two years, please provide proof of those results. Ensure your results include your name, the date of the test, and the results.
